Volunteer Liability Waiver Forms

Volunteers must fill out a waiver of liability in order to volunteer on site. We will have plenty of copies there, but filling them out ahead of time may save you some time and hassle once you get to the site. If you bring it with you, please make sure you fill out both pages completely and sign it. You will need the free version of Acrobat Reader to view and print the forms.

Anyone 18 years of age or older should fill out this version of our liability form. Make sure to fill out both pages completely and sign it.
Download a Minor Waiver
Anyone 16-17 years of age should fill out this version of our liability form. Anyone under 16 years of age is not allowed to be on site.
